Shoulder tendon transfer surgery is a surgical procedure to restore shoulder function when there is significant rotator cuff damage. In this procedure, a healthy tendon from another part of the body, usually the biceps or the latissimus dorsi muscle, is used to replace the damaged rotator cuff tendon. The new tendon is surgically attached to the shoulder bone to help the shoulder joint move correctly. This procedure is generally used in patients who have chronic rotator cuff tears or who have had previous unsuccessful shoulder surgeries.
There are several types of shoulder tendon transfer surgeries, including the L'Episcopo procedure, the Eden-Hybinette procedure, and the Gerber procedure. The L'Episcopo procedure involves the transfer of the teres major tendon from the back of the shoulder to the front of the shoulder. The Eden-Hybinette procedure involves the transfer of the latissimus dorsi muscle from the back to the front of the shoulder. The Gerber procedure involves the transfer of the subscapularis tendon from the front to the top of the shoulder.
The L'Episcopo procedure is usually used in patients who have had previous shoulder surgery, while the Eden-Hybinette and Gerber procedures are typically used in patients with large rotator cuff tears. The choice of procedure depends on the size and location of the rotator cuff tear, as well as the patient's age, overall health, and activity level.
The surgery is performed under general anesthesia, and the patient is typically hospitalized for one or two days. During the surgery, the surgeon makes an incision in the shoulder to access the rotator cuff. The damaged tissue is removed, and the healthy tendon is prepared for transfer. The new tendon is then attached to the bone in the shoulder using sutures and/or metal screws. After the procedure, the patient's arm is placed in a sling to help support the shoulder as it heals.
