High Tibial Osteotomy (HTO) is a surgical procedure that is used to treat knee arthritis, specifically when there is wear and tear on one side of the knee joint. This procedure involves making a cut in the tibia bone (the shin bone), and then realigning the leg so that weight-bearing forces are shifted from the damaged part of the knee joint to the healthier side. HTO is considered a joint-preserving surgery because it aims to slow down or prevent the progression of arthritis, and is often considered as an alternative to total knee replacement.
There are several types of HTO surgeries, but the most common is called a medial opening wedge HTO. In this procedure, a wedge-shaped piece of bone is removed from the outer side of the tibia bone, and the bone is then opened up like a book. The gap is then filled with a bone graft, and a plate and screws are used to hold the bone in place. This opens up the inside part of the knee joint, where there is more healthy cartilage, and reduces the pressure on the damaged part of the joint.
Another type of HTO is a lateral closing wedge HTO, where a wedge-shaped piece of bone is removed from the inner side of the tibia bone, and the bone is then closed like a book. This is used less often, as it can cause more instability in the knee joint and has a higher risk of complications.
HTO surgery is typically recommended for patients who are younger, active, and have early to mid-stage knee arthritis on one side of the joint. Patients who are overweight, have advanced knee arthritis, or have arthritis on both sides of the joint are not good candidates for this surgery.

Recovery from HTO surgery typically takes several months. Patients will need to use crutches and wear a brace for several weeks after surgery, and will need to participate in physical therapy to regain strength and range of motion in the knee joint. Full recovery can take up to a year.
As with any surgery, there are potential risks and complications associated with HTO. These can include infection, blood clots, nerve damage, and improper healing of the bone. However, the risk of complications can be reduced by carefully following post-operative instructions and by selecting an experienced and qualified surgeon.
